Administering Medicine

We will only administer medicines with the consent of the parents or carers and with a completed and signed form.

Absence Requests

Although we are unable to authorise leave for holidays, we can authorise leave for exceptional circumstances. In either case, we ask that you complete this form if you wish to take your child out of school.
